ISS-Survey The PHC department should have the following staff (cadre): Director PHC, Head Training & Manpower Development, Head Planning/M&E, Heads PHC Services Division, Head Nutrition, WATSAN, Head, Essential drugs Each staff worth 2 points) The state should have the plans (workplans) of all the LGAs stored in a safe and accessible plac). > The current work plan available and approved > Verify the last two years work plans The state should have a M&E team: > It should be multidisciplinary > Current quarter activity plan available > Reports of previous year and last two quarters available The state conduct monthly LGA supervisory visits > The supervisory visit follow the schedule > The supervisory visit is regular and/or follows the identified gaps from previous visits > The report of the supervision is available > The report is circulated: NPHCDA zonal office, SPHCDA, SMoH, partners, etc Its all or nothing The state documented the health sector human resource profile by facility and by cadre. > A list and number available plus number needed The state has record of deployment of health workers > Deployment and/engagement is based on the need as identified from gap analysis Supervisor check the last employment/deployment batch with the gap analysis > The state has --> training plans --> records/reports of training conducted for PHC workers in the last quarter
